IDENTITY
The Challenge: Design a portal for Cincinnati Children's Hospital and Medical Center (CCHMC) to share key information about youth in foster care between Hamilton County Job and Family Services. The portal will allow caretakers from both organizations to access and share medical and legal data, streamlining the information sharing process.

The Timeline: May 2017 - August 2017
Currently, information transfer of youth in foster care between CCHMC and HCJFS happens via an analog process of calling and requesting information. Requests are not always answered in a timely manner, creating delays in care and placements for youth in foster care. IDENTITY directly connects CCHMC and HCJFS employees with this information to provide the best care for youth in custody.
18 professionals, 8 from HCJFS and 10 from CCHMC ranging from clinicians to ongoing caseworkers, participated in a series of co-creation activities. The stakeholders completed two interactions as part of the research phase of the project: schedule mapping and card sorting.
The maps of each stakeholder’s schedule gave insight into what each stakeholders does, what tools they currently use in the field, and at what points in their day they might use the IDENTITY portal.
The card sorting activity was used to categorize information for an efficient and easy to use architecture for the site.
Each stakeholder was tasked with navigating the various wireframes to test for efficiency and usability. While the stakeholders moved through the prototypes, they were questioned about functionality, comfort, and access to information. The team learned that stakeholders wanted the ability for multi-search fields for finding the correct youth’s data, bookmark navigation for ease of reuse, and an aesthetically pleasing data organization system.
The final form of the IDENTITY portal executes the tangible and intangible insights, creating a dynamic layout that satisfies all stakeholder needs. Below is a link to the InVision prototype of the site and a Vimeo video highlighting the interactive features of IDENTITY.

The product has been launched in Hamilton County, Ohio, for further refinements and testing.
